Do I need a doctor's referral for physical therapy in Colorado?
No. Colorado is a direct-access state, so you can schedule with a physical therapist without a referral. Some insurance plans still require a referral or prescription for coverage. Medicare does not. If you're unsure, call us at (970) 884-2423 and we'll verify your benefits before your first visit.
What insurance do you accept?
We accept Medicare, Health First Colorado (Medicaid), and most major commercial plans. See our insurance page for the current list. We verify benefits before your first visit and tell you what to expect for copays or deductibles.
What if I don't have insurance?
We offer straightforward self-pay rates. Call for current pricing.
What should I bring to my first appointment?
Photo ID, insurance card, any referral or prescription, imaging or surgical reports if you have them, a list of medications, and comfortable clothing you can move in. Arrive about 15 minutes early to complete paperwork, or download the forms ahead of time.
How long is each visit?
Your evaluation is about 60 minutes. Follow-up visits are typically 45–60 minutes, one-on-one with your physical therapist.
How many visits will I need?
It depends on your condition and goals. Many patients improve significantly in 4–8 visits. Post-surgical and chronic conditions take longer. We set clear expectations at your evaluation and adjust as you progress.
Will I see the same therapist every time?
Yes. Consistency matters, so you work with the same licensed physical therapist for your entire plan of care whenever possible.
Is Pine River Physical Therapy still open?
Pine River Physical Therapy has merged with Bayfield Physical Therapy. We're at the same location — 480 Wolverine Dr, Suite 5 — under the Bayfield PT name, and Paula Mooney, PT, continues to see patients here. Both phone numbers still work. Learn more about the merger.
Do you treat patients from Ignacio and Durango?
Yes. We serve all of eastern La Plata County — Bayfield, Ignacio, Vallecito, Forest Lakes, Gem Village, and east Durango. Many patients who have surgery in Durango or Farmington do their rehab here to stay close to home.
What conditions do you treat?
Most orthopedic and neurological conditions: back and neck pain, joint pain, sports injuries, post-surgical rehab, arthritis, balance and dizziness problems, and chronic pain. See conditions we treat.
What is your cancellation policy?
Your appointment time is reserved for you. Please give at least 24 hours' notice to cancel or reschedule so another patient can use the time. Late cancellations and no-shows may be subject to a fee.
Where are you located?
