Step 1: Schedule
Call (970) 884-2423 or request an appointment online. You do not need a doctor's referral to see us — Colorado is a direct-access state. Some insurance plans require one for coverage, and we'll tell you if yours does.
Step 2: We verify your insurance
Before your first visit we contact your insurance company, confirm your benefits, and let you know what to expect for copays or deductibles. No surprises. Plans we accept →
Step 3: Paperwork
Please arrive about 15 minutes before your first appointment to complete your intake paperwork. If you'd rather fill it out at home, call us at (970) 884-2423 and we'll email the forms to you.
What to bring
- Photo ID and insurance card
- Referral or prescription, if you have one
- Imaging reports (X-ray, MRI) and surgical notes
- List of current medications
- Comfortable clothing you can move in — shorts for knee/hip, a tank top for shoulder/neck
Your first visit
Plan on about an hour, all one-on-one with a licensed physical therapist. We'll talk through your history and goals, assess how you move, and start treatment the same day. You'll leave with a clear plan, your first home exercises, and a realistic timeline. More about the evaluation →
Cancellations
Please give 24 hours' notice to cancel or reschedule so another patient can use the time.
